5th Grade Mainstreamed

Instructional Modifications

  • An itinerant teacher of the deaf will provide auditory training and direct academic instruction weekly.
  • The school district will provide auditory trainer for use at school.
  • Preferential classroom seating will be utilized.
  • Teachers will face the student when speaking.
  • Teachers will point to the speaker in group discussions.
  • Teachers will restate what the speaker said in group discussions.
  • Teachers will avoid undue movement while instructing.
  • Teachers will avoid standing in front of a window or other source of bright illumination.
  • Closed captioning on all classroom televisions will be utilized.
  • Written outlines will be utilized for instruction.
  • Study guides will be provided for test preparation.
  • Written journal will be utilized for exchanges between student, teacher and parents.
  • Written journal will include a brief description of next weeks lessons.
  • Summary or outline of films, videotapes and filmstrips that are used in instruction are to be provided to the child.
  • An itinerant teacher of the deaf will evaluate the classrooms for excessive background noise and reasonable accommodations will be made to minimize distractions.
  • An itinerant teacher of the deaf will be observe the student during class and offer communication strategies to the classroom teacher.
  • A set of textbooks will be issued to the student to be maintained at his home during the school year.

Goals

Student will increase skills in the Science essential elements at the 5th grade level with 70% accuracy as measured by daily work, tests, performance and/or behavior.

Student will increase skills in the Social Studies essential elements at the 5th grade level with 70% accuracy as measured by daily work, tests, performance and/or behavior.

Student will increase skills in the Health essential elements at the 5th grade level with 70% accuracy as measured by daily work, tests, performance and/or behavior.

Student will develop skills in communication processes of reading.

  • Will develop vocabulary and word recognition at the 5th grade level at 70% accuracy as measured by daily work and tests.

  • Student will increase skills to read critically at the 5th grade level at 70% accuracy as measured by daily classroom performance.

  • Student will increase ability to enjoy reading and appreciate literature at the 5th grade level at 70% accuracy as measured by daily work and tests.

  • Student will develop skills in communication processes of English. Will listen actively at the 5th grade level at 70% accuracy as measured by daily work and tests.

  • Student will speak effectively at the 5th grade level at 70% accuracy as measured by daily classroom performance.

  • Student will write for varied purposes at the 5th grade level at 70% accuracy as measured by daily work and tests.

  • Student will write effectively at the 5th grade level at 70% accuracy as measured by daily work and tests.

Student will develop skills in communication processes of spelling.

  • Student will increase skill in understanding numbers and the place value system at the 5th grade level at 70% accuracy as measured by daily work and tests.

  • Student will use numbers in basic operations at the 5th grade level at 70% accuracy as measured by daily work and tests.

  • Student will experience problem solving opportunities at the 5th grade level at 70% accuracy as measured by daily work and tests.

  • Student will explore measurement concepts at the 5th grade level at 70% accuracy as measured by daily work and tests.

  • Student will discover relationships with geometric experiences at the 5th grade level at 70% accuracy as measured by daily work and tests.

  • Student will demonstrate knowledge of representing numbers on a number line at the 5th grade level at 70% accuracy as measured by daily work and tests.

Speech and Auditory Goals

  • Student will improve vowel discrimination and identification at 70% accuracy as measured by teacher made tests.

  • Student will discriminate and identify consonants and consonant blends in the initial and final position of words at 70% accuracy as measured by teacher made tests.

  • Student will do the above two skills in the presence of medium to loud background noise at 70% accuracy as measured by teacher made tests.

    Student will demonstrate measurable progress toward the acquisition of developmentally appropriate vocabulary and related concepts.

  • Student will define target words chosen from specific vocabulary categories.

  • Student will use target words in a sentence.

  • Student will demonstrate developmentally appropriate conversation as needed for personal/social control.

  • The student will demonstrate developmentally appropriate auditory discrimination, sequential memory, and/or recall.

  • The student will demonstrate measurable progress towards the acquisition of developmentally appropriate phonological processes: articulation of phoneme S.

Student will demonstrate problem solving strategies by offering solutions.
